Cloud Care Services works in partnership with General Practitioners, pharmacies, and allied health providers to support older people and NDIS participants to access coordinated, timely, and person-centred healthcare within their homes and communities.

Our approach reflects the shared principles of person-centred care, choice and control, independence, safety, and outcomes-focused support, in alignment with the Support at Home program, Commonwealth Home Support Programme (CHSP), and the NDIS Practice Framework.

Coordinated Care Planning

Support is delivered in line with an agreed care plan or NDIS participant plan, with services coordinated alongside GPs, pharmacists, and allied health professionals to reflect assessed needs, goals, and preferences.
