Supporting Independence at Home and in the Community

Personal Care under Support at Home, CHSP & NDIS

Cloud Care Services delivers community-based personal care and supports that enable older people and NDIS participants to live safely, independently, and with dignity in their own homes and communities.

Our services are guided by the shared principles of person-centred care, choice and control, independence, safety, and outcomes-focused support, in alignment with the Support at Home program, Commonwealth Home Support Programme (CHSP), and the NDIS Practice Framework.

Person-Centred, Goal-Directed & Participant-Led Care

Individualised Support Planning
All care and supports are delivered in line with an agreed care plan or NDIS participant plan, developed collaboratively to reflect assessed needs, goals, preferences, and cultural considerations.

Wellness, Reablement & Capacity Building
We support clients and participants to maintain, improve, or regain functional capacity wherever possible — encouraging independence and active participation in daily living.

Choice, Control & Dignity
Clients and participants are supported to make informed decisions about how, when, and by whom their care is delivered, with respect for privacy, autonomy, and personal routines.

Safe, Quality & Reliable Support Delivery

Safe & Compliant Practice
Services are delivered in accordance with safe work practices, manual handling guidelines, infection prevention and control requirements, and organisational risk management processes.

Qualified & Supported Workforce
Our care workers and support staff are appropriately trained, screened, and supervised to meet the requirements of aged care and NDIS service delivery, including ongoing competency development.

Consistency & Continuity of Care
Where possible, we prioritise continuity of staff to build trust, familiarity, and positive care relationships.

Safeguarding, Rights & Wellbeing

Risk-Aware, Not Risk-Averse
We support positive risk-taking that enables independence, while identifying and mitigating foreseeable risks.

Safeguarding & Incident Management
We are committed to preventing abuse, neglect, exploitation, and restrictive practices, with clear reporting, escalation, and response processes in place.

Responsive & Coordinated Support
