    ASSISTANCE TO STRENGTHEN PEOPLE’S ABILITY TO COORDINATE

    Cloud Care Services provides assistance to strengthen people’s ability to coordinate and implement their NDIS supports and participate more fully in the community, through support coordination services.


    PAYMENT PROCESS AND OUR PRICING

    Cloud Care Services provides NDIS Plan Management, supporting the financial management of NDIS Participant’s Plans, including making payments to providers, processing expense claims, providing monthly statements for NDIS participants and claiming for payment from the National Disability Insurance Agency (NDIA).


    ASSISTANCE WITH LIFE STAGE TRANSITIONS AND SKILL DEVELOPMENT

    Assistance with life stage transitions and skill development, including assistance with accommodation and tenancy obligations, as well as budgeting, mentoring, peer support, individual skill development, and decision making to support people to make transitions on their own terms and in a way that meets their goals and aspirations.

    INDIVIDUAL AND GROUP COMMUNITY PARTICIPATION ACTIVITIES

    Cloud Care Services provides individual and community participation activities, including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and building confidence and social skills.


    INDIVIDUAL OR GROUP TRAINING AND DEVELOPMENT ACTIVITIES

    Cloud Care Services provides individual or group training and development activities undertaken by the NDIS Participant or their carer to increase the NDIS Participant’s ability to live as autonomously as possible. Skills training includes personal skills; living skills; training for carers / parents; NDIS planning and plan management; community independence, social and recreational situations; skills for household tasks; public transport training, support and social skills.

    The Home Care Packages program is a government-subsidised program that provides long-term support for older people who want to stay living at home.

    There are four levels of Home Care Packages for different levels of care and support needs. Each level receives a different amount of funding. This money can be used to purchase hours of care or other support that suits your needs.

    Home Care Package funds are not paid to you directly. Instead, you choose an approved Home Care Package provider to administer the funds on your behalf and give you ‘case management’ support and advice. Once you have a Home Care Package, you can keep it for as long as you need. If your needs increase, you may be re-assessed for a higher-level package.

    There are four levels of Home Care Packages for different care and support needs:

  • Level 1 for basic care needs;
  • Level 2 for low care needs;
  • Level 3 for intermediate care needs; and
  • Level 4 or high care needs.
  • Each level receives a different amount of government funding. The higher the level, the more funding the government pays on your behalf, allowing you to purchase more services. A Level 4 package allows you to purchase the most services. Once you’ve had an Aged Care Assessment, you will be told which level applies for you.

    Home Care Packages are not the only way you can have your needs met. You may find extra support from your family, in your community, in your social network, using your own money or resources, or by creating a network of people that may be able to help you. There are also lots of other programs, some of which are funded by government, which you might be able to use.
