Aged Care

HOME CARE PACKAGES: LEVEL 1 (HCP LEVEL 1)

Home Care Packages are in-home care services that are funded (either in part or in full) by the Australian Government. Level 1 Home Care Packages (HCP Level 1) are designed to support people who have basic care needs. Basic care needs can include basic everyday tasks including:

Domestic assistance;

Meal preparation;

Personal care;

Shopping;

Social support; and

Transportation.

HOME CARE PACKAGES: LEVEL 2 (HCP LEVEL 2)

Home Care Packages are in-home care and home help services that are funded (either in part or in full) by the Australian Government. Level 2 Home Care Packages (sometimes seen abbreviated as HCP Level 2 or HCP2) are aimed at supporting people who have low care needs. If you or a loved one has low care needs, you may be eligible to receive assistance with:

Basic home & garden maintenance;

Domestic assistance;

Meal preparation;

Personal care;

Shopping;

Social support;

Some nursing care; and

Transportation.

HOME CARE PACKAGES: LEVEL 3 (HCP LEVEL 3)

Home Care Packages are in-home care and home care services that are funded (either in part or in full) by the Australian Government. Level 3 Home Care Packages (HCP Level 3 or HCP3) are designed to support people who have intermediate care needs. If you or a loved one has intermediate care needs, you may be eligible to receive assistance with:

Domestic assistance;

Meal preparation;

Medication management;

Personal care;

Shopping;

Social support;

Some nursing support;

Support with changes to memory and/or behaviour; and

Transportation.

HOME CARE PACKAGES: LEVEL 4 (HCP LEVEL 4)

Home Care Packages are in-home care and home help services that are funded (either in part or in full) by the Australian Government. Level 4 Home Care Packages (HCP Level 4 or HCP4) are intended to support people who have high care needs. If you or a loved one has high or complex care needs, you may be eligible to receive assistance with:

Allied health support;

Assistance with aids and appliance;

Domestic assistance (such as laundry/cleaning etc);

Meal preparation;

Medication management

Personal care (such as showering/dressing etc);

Shopping;

Social support;

Support with changes to memory and/or behaviour; and

Transportation.

NDIS

HOUSEHOLD TASKS INCLUDE:

General housework;

General yard cleans;

Laundry;

Meal preparation;

Prescription delivery;

Rubbish removal;

Shopping and errands;

Spring cleans;

Squalor cleans;

Transportation; and

Window cleaning.

ATTENDANT PERSONAL CARE

Assisting with mobility and exercises;

Bathing and personal hygiene;

Dressing and grooming;

Medication prompting and administration;

Palliative care;

Peg feed and medication administration, bowl care, etc;

Skill retention and development;

Specialised services; and

Welfare visits

COMMUNITY NURSING CARE

Cloud Care Services’ team of highly skilled and qualified nurses offers a range of services to assist with your healthcare needs including:

Chronic Disease Management: working with you and your doctors to find ways to manage your condition at home;

Diabetes Management: assistance with care planning, administering insulin, wound care, and clinical monitoring;

Wound Management and Stoma Care: help to dress and manage wounds, and stoma maintenance;

Continence Management: assessment and care plans to help you maintain your lifestyle; and

Medication Management: our nurses help you to manage your medication, including injections, tablets, and creams.

Rehabilitation Services

Cloud Care Services can provide training and development across a range of life skills, including:

Personal hygiene: such as showering, toileting and personal style;

Nutrition: such as shopping for groceries and planning for and preparing a healthy meal;

Maintaining cleanliness in their personal space: such as housework, laundering of clothes & addressing maintenance issues or repairs

Communication: with service providers and support networks; and

Transport and travel: such as planning their journey, accessing public transport and trouble-shooting issues.

Transportation

Cloud Care Services provides transportation for an NDIS Participant to get to school, medical appointments, dental appointments, education, employment, and community activities when they are unable to travel by other means.

Group & Centre Based Activities

Cloud Care Services provides assistance to NDIS Participants to participate in group-based community, social and recreational activities & many more.

Plan Management

Cloud Care Services strives to help you achieve your goals by managing your National Disability Insurance Scheme (NDIS) funds, including making payments to providers, expense claims processing, providing monthly statements to participants and claiming for payment from the NDIA.
